Been reading some of your posts and I'm impressed with what been done so far but I thought of some things I wanted to mention:
Instead of running out of time being an immediate game over / fail state perhaps have the difficulty increase:
Additional NPC's spawn and all NPC's gain better visual detection ability seeing as its daytime ... So not to make this an an infinite loop perhaps the character could faint from exhaustion if out for to long (maybe an extra 5 minutes on easy difficulty, 2 and a half minutes on normal difficulty and a 1 minute on hard difficulty)
I figure you have already considered multiple options to accelerate the growth of the character arousal - sex toys and specific clothing (using the term clothing loosely here - anything that can technically be worn does not necessarily need to cover up anything ... like shibari as an example) but perhaps remaining fairly close to NPC's back in revealing / no clothing could cause it to increase faster.
Ran into some problems with the 'find your clothing' scenario:
The black shirt spawned on a dark colored patch of ground which meaning finding it was more luck then anything else - perhaps have the clothing spawn draped over objects instead or have something that makes them easier to find when you get close like the animation around the keys item in the 'find your keys' scenario.
Another bug in the 'find your clothes ' that occurred was if you find a character over outfit piece and wear it first then you cannot equip the under outfit item if you find it later - in this case I found the skirt item before the panties and could not equip the panties or remove the skirt to equip the panties followed by reequipping the skirt ... guessing an unexpected logic interaction as normally the character has to strip the outer layer before the under layer.

Jaguar26 I appreciate this game you are building, and I think it is amusing, although currently, it feels a bit lacking in direction... although it may just be that I can't seem to locate the cameras around town so that mode feels lacking to me. I thought of a mode of gameplay that might be interesting, and possibly not too difficult to implement.
I think it would be interesting if there were locations that wanted a specific article of clothing placed there to fill a progress bar. They could require any item of clothing you wore out of the house, or even clothing that you may have to look around to find and borrow from your neighbor's clothes lines or other places. More articles placed in the "proper" locations could build the bar faster. The wrong article in a location may penalize your progress after a warning and a short grace period. A location that does not have its desired piece of clothing could also alert the player, and then halt progress until that is corrected.
After some time of having the proper article in them, the locations jump to other parts of the town, or swap the clothes they need to be "activated" with other locations. In this way, you could have an ever changing goal that keeps shifting and compels your character to run around town dressing and undressing while playing a bit of a memory game usually reserved for only the sluttiest of people "Now where did I leave my underwear? In the park? No, that's where my shirt is..."
The goal is simply to fill the progress bar. Extra difficulty could be added as you get closer to filling the progress bar with increasingly more challenging nudity zones that penalize you greatly for entering unless topless, bottomless, or nude.
TL/DR: Perhaps a new game mode akin to controlling parts of the map to win a Domination map in an FPS, only instead of a squadmate being needed to control an area and gain points towards winning, you need to leave your panties.

There could even be a mode where she sets them vibrating and heads out to complete given tasks but leaving the controller at home. There'd be a timer to complete the task before she uncontrollably orgasms in public. Could work for any of the scenarios like Get Dressed/Find Your Clothes or Locked Out.
I'm not sure about fainting, but getting caught resulting in more people coming out to look for you does sound interesting.
Have the zone change in that the area is now filled with people who know that theres a naked girl running around and are excited by it.
Switch from the NPCs passively roaming around to a bunch of people actively looking for you and update your objective to get back to your apartment before you get caught (your embarrassment meter/hp refills when the new npcs spawn, but if it drains a second time, you get the game-over)
Have the second tier of NPCs... call for their friends when they spot her , or try to take pictures of her or something.

-Scared and ran away from the hero.
-Frightened and called the police (the player's goal is to run and hide from them).
-He left and photographed the hero and then he left the hero (post the photo in the news * see below).
- Get interested and watch how the hero undresses.
I suggest adding a variety of bots.
-Women for a change.
-Police officer to arrest the hero.
I suggest adding news to the press.
Photos of the hero will be published when he gets naked.

I've played a few exhib games where the po-po were able to catch you and game over you, might be a nice idea but I would only keep them in one game mode, maybe two but not all. Sometimes you just wanna flash npcs and see their reactions before running away
There was another game where the char posted their own face-hidden self-taken exhib pics on a streaker website as part of the story, so that's always a nice idea. Might be harder to pull off with a non-event-based 3d game though vs 2d pre-written events/art.
Right now my personal biggest want is body paint and/or more outfit options And a way to move the camera to look around during masturbation, or even a first-person view with limited side-to-side glancing (and of course, looking down at your body for a small bonus to excitement)
