But we know she feels insanely guilty about being with MC, so maybe she's beating herself up about whatever happened with Jill—no matter how small—out of guilt, blowing it out of proportion.

Bella still feels married but still that feels like a odd reaction to something Jill did in her sleep, I think it would have to be something more then that unless it started out as Jill sleep humping her but Bella woke Jill up and it turn it to something more then it would make sense to me

The game is being narrated by MC at some point in the future. So, what the player sees is what the MC envisions while reading the diary. MC is a pervert, thus mommy dearest is frequently nude.
I guess I never saw it as that at all when it comes to the diary. For me when the diary comes up I just imagine it's the MC reading it but the flashbacks aren't his thought or imagination but just a flashback in general
