When we have sometimes choice "Peek" or "Don't take the risk", is there really a risk (or it's more about roleplay) ?

There are occasionally risks, I think a few of the times it happens the girl might catch you and dislike you - but also some of them notice and are into it. Try it and see if they react! You can always roll back.

From what I remember, there aren't any girls whose route outright ends if you peep on them. Even Bella. If you do it constantly and they don't like it you might eventually lose enough points with them to miss a threshold, but I don't think it's likely.

Tommy's attitude during the whole game makes clear he has a crush on her but never has her way with her. Knowing the man he would have bragged about it, instead he's just stunned when he learned that Tremolo fucked her on the roof. Besides if you can find a single scene where Quinn seems to show any interest in him from a sexual or romantic point of view ?

It makes sense with his character and his absolute hatred of Vinny. He never was the alpha male and was always overshadowed by Vinny, then the MC who both did what he couldn't.

More of a leader than the wimpy Rusty. More of a bro than the troubled Derek. Tommy is always interested in how he's doing with the other girls. It's a shame they didn't show us the full episode with Tommy and Pink Rose when they got drunk.
Like any person, Tommy has his flaws. Even though he envies Tremolo's leadership, fights with him, and doesn't want Darkwen to sleep with his sister, he's a bigger bro to me than Derek.

And in this case, he simply accepted that Quinn is now with Tremolo and didn't compete with him. A real "DIK".

Also, which are the actual decisions that matter? It's looking like I am going to have to start a fresh run from the begining and I want to get it right.
I have tried going back to my throuple save at the start of episode 9 and making all Josy decisions after that point, but it still gave me the Maya scene!

Wow, you must really hate Josy huh?
Even on my playthrough where I tried to choose evenly between them, I still got Josy scene.
Pretty sure this counts choices between them all the way back to episode 5, where you choose who to follow during the party at the Preps.

There are a ton of choices that give either plus or minus RP to Josy and/or Maya from ep3 all the way to ep10. This decides which scenes you get, and also if you can say I love you by getting enough points for one of the 2 chars.
