Given the dialogues in the game lately i think Ann knows the whole "carpal tunnel" thing is bullshit that Max and Kira came up with. The dialogue not updating in the massage scene is one of those continuity issues, like how until recently Lisa got mad at Max for entering their room when she was changing, even if Max was her boyfriend.

Here's how I see it going:
Ann takes Max to the doctor about his carpal tunnel syndrome, because of how long it's been going on. Somehow, it turns to the subject of Max's erections. Maybe the doctor talks about it being from repetitive motion, and from there it segues into a discussion of how he seems to be hard all of the time, which Ann awkwardly avoiding mentioning how she jerks him off sometimes. Max just happens to get hard during all of this because of Ann and the doctor. Mix in a little porn-movie logic, and now Max has no pants off. Maybe she offers to show Max how to masturbate in a way that won't aggravate his carpal tunnel. Or she writes a prescription for Ann to keep jerking and sucking him off. Kira wants to make Ann jealous and convinces her to take Max to the doctor bc of his abnormal carpal tunnel. Kira told her that a check-up is mandatory in the porn biz.
