I think you have to step in during the event where Raven is first talking to the guy in the alley. When she tells you not to intervene. Also, I think it's just a scene, a bad ending.
The choice includes a fairly clear advice of where you are going: :

Yeah that's what I did and I'm p sure it said going through with it made it unavoidable but it just never happened for me. The rest of the game played out like normal.

That's because you completed the game without hitting the prerequisites for it to trigger, which is getting most character to 4 hearts.
