Ok soo, this needs Multiple Protagonist and Female Protagonist Tag, during the phone call from the mc to Sidney (?) the choices were for her(if this is going to be normal those tags are needed) also that was quite annoying, 13 times clicking on the same choice "Speak your truth" WTF.....
Thela even being a hallucination, looks like the trope of that "best friend" who is very annoying, trying to teach how the mc should behave or that he spoke badly to the sister and bla bla bla and as soon as mc enters the gas station and sees this other girl the first thing she says is "She looks like a cum bucket" ahhh but the sister who is a porn actress doesn't look like that? how good it would be to have a choice to tell her to shut the fuck up...
The back story that the dev added to the sister is hilarious, she ghosted the mc for 6 years, then in a game she tells him that she is a porn actress at least the mc didn't find out with a video of her in a GangBang
And for those who have doubts, I think she was always introduced as LI, that is, she is in the banner caressing the mc, the dev never ruled out the Incest tag (even if it was fake shit-cest) and also he had already said that the mc could have a relationship with all the girls that appear.....

Wish these people stop being APOLYGIST for shitty writing of characters, Especially family members.. step cast or not.. people are SENSITIVE when it comes to that so it's important to write them with care and not make them been raped, sluts, cam girls. etc...
And like I said even if she is not a LI, the typical slut trope is tired df out and people are tired of it.

Not that bothered by what the stepsis does in her life, but it was a major bummer finding out that the story doesn't progress any further than the last update. Downloaded the update without checking the changelog because I remember it was one of the most well put together games I'd played here, turns out that was a mistake.
It's one thing doing this during internal development of a game to be released down the line, but another for episodic releases. We're always looking for progression, some forward momentum, and adding to the prologue instead doesn't satisfy that. Projects like these need roadmaps before the grunt work starts, or we end up with unnecessary feature creep, and narrative elements being added out of order, like so.

To be honest, the MC is accompanied by and interacts with his imaginary ex-girlfriend, he ends his therapy and we see he is already low on medication and takes only pills when he really thinks it's needed and he already knows he will run out soon.
With a title like "Derealisation" (getting detached from reality?) it's safe to assume MC already has a twisted view of his surroundings.... He already is (bordering) psychotic, he's smart enough to still appear "normal" in most situations, but he will increasingly start losing touch with reality from medication deprival.... (psychotic people being suspicious is pretty much the standard, so is this a sign of the clinical picture or not?)
I don't want to justify the dev writing the sister into a pornstar role, but i seriously wonder how much of all we see is real and what exactly is just in MC's head? Last 2 updates mostly added the madness iirc... First the imaginary ex, now the pornstar profession....

This update is something like an extended prologue, where the fucked up mental state of the MC is better revealed. I like it

I'll be honest, the second the sis said she was in porn I stopped caring about the story and started fast forwarding to see if the renders were worth watching. The second a potential LI drops the I fuck a ton for fun it instantly loses me.

I think you should have been able to choose how MC reacts to the sister. The way it's handled now is kind of like a joke which is a bit weird. Some people just don't like this kind of stuff ESPECIALLY with a sister. Also if she's an LI then this whole situation is just a recipe for disaster, if she's like a minor side character then it's kind of whatever imo.
