Zero. It just changes dialogue so the first elf woman is the MC's mother. That's it.

You do get a couple, like the naked bath scene or waking up next to her with her clothes off, probably 5 or so extra scenes

Those are extra? Huh. I figured they were just a part of the normal game flow, given that there's no "mother" required to have them still make sense. Seems silly that they're patch scenes and not just general ones.

i'm pretty sure the patch doesn't change any dialogue. just adds the scenes/ options. Even without the patch they're still related.

Is the dev pulling a Komisari and claiming to work on the game but is just milking the fans with false promises?

The dev used to be a student so he had time to work on the game, he's found a full time job so now he only works when he can
Considering there's something like 50 paid subscribers total across Patreon and SS for this game, i think you have vastly exaggerated idea of what "milking" here would produce.

Yeah he's just one of those devs that doesnt post much and it doesnt help that he tends to work at a slower pace for numerous reasons. Think of him like HopesGaming whos creating DeLuca Family and has like 2patreon posts a year. Same deal. Some devs are super involved and post a shit ton other are like ghosts and only make posts when the update is ready.

"The next update won't be out before may/june 2025" is, shall we say, relatively good news.
It'll make it like 1.5 year since last update. But likely longer.

Considering I am enjoying this game for free, I will gladly wait.

I completely agree, the dev is doing it on his free time and it's quality art with good writing. I'm happy to know the game isn't abandoned.

A this point, only an update about progress would've sufficed. The rules are that it's over 3 months without any news whatsoever or that it's been more than 18 months, regardless of communication by the dev. So there are still 6 months for that second threshold. That said, once Abandoned is already on a game, it will not be removed until an actual substantive (i.e., not just a proofreading update or w/e) update is released.
