To be honest not sure , but from what I have heard it largely depends on how the game was created in the first place. From what I have heard if the game was censored after it was created then removing the censoring is not as hard if it was censored during the creation of each art work then it would have to be re-drawn. Perhaps someone more familiar with the process can enlighten us about how it works.

A lot of games from Japan are censored during the arts creation meaning it can't just be uncensored it's part of the actual art, it needs to be completely redrawn which takes a LOT of work.
JAST and Mangagamer have done this for some games in the past but due to cost and time don't do it very often.
The censorship is done this way because of how after market censorship is bypassed, it covers the devs arse so the don't get in legal trouble.
The censorship added after the fact is pretty easily removed by people that know what they are doing. Games censored like this usually have it removed when selling in the west though not always.
Because censorship removal doesn't drastically affect sales it's not something that's seen as a priority.

theres a difference between "porn with rpg" and "rpg with porn". i didnt play the japanese version of evenicle 2 so i wont know but evenicle 1 is rpg with porn as a bonus.
as for the censorship. im sure some of many of us wont mind the mosaic censorship but its a different story if the scene and/or writing gets censored.
as for the mosaic, im sure theres a program from reddit that decensor it provided that you have the image(?)
