Enjoyed version 2.5 and believe it has potential to be a fun ero game and some good NTR as well. Though I have some suggestions which you may have already considered. Anyway here is my suggestions and rant.
Lets start with the one you seem to be going with already. That being a timer, but one that is based on your adventure progress. Meaning obviously that a player has a set number of days to complete each adventure or quest. Though this will not do by itself and for that reason I also suggest the addition of a ero progress blocker. Meaning that it would be impossible to get over 100 affections points for both Kazuma and John during the first quest as an example score. Combined this will incentives players to play the RPG element of the game, allow for main story scenes to be introduced before certain H-scenes, and allow you to pace the H-scene progress. For example this could allow you to build quest items or events into a H-scene.
As a final suggestion on this point, at the end of the quest you can have certain main story events which show either Kazuma or John growing closer with the girls in a plot focused way. This could be a flavor scene purely to see the girls grow closer in a more organic way (Talking about the quest, with new characters, or interacting with items found on the quest) or could have some game mechanic effect such as opening the girl up for higher affection development during the next quest. Overall I believe the goal should be to build a overarching narrative that connects the H-scenes to the core story and character development. As well as stop players from simply never leaving the mansion as I don't currently see a real reason to leave the mansion when we can get all the H-scenes without having to.
On that point, H-scenes should ideally be slowed down a little. I understand why the H-scenes moved so fast as you have to grab people's attention fast to get subscribers and an audience, but rushing to sex is almost always a bad idea in NTR. Sexual tension is the name of the game and so I suggest beefing up the H-scenes so as to have multiple foreplay scenes before we ever get to penis reveals let alone sex. This will help build tension as well as add padding to the NTR development. Ideally I'd like to see the heroines being seduced, during the foreplay, with one idea being that they find John pleasing to be around because he acts more like old Kazuma than new Kazuma does, but a stronger cooler version. As Kazuma sexually harassed and bullied the girls in lore as well but was a coward about it. This could explain there fall for him a little better besides being purely a sexual domination due to big penis (Though I have no issue with that being a big reason).
Next, the penis comparison system can be more fleshed out. To begin with the addition of purely flavor texts which comes up during the H-scene after the heroine has seen the other parties penis. Next, the addition of repercussions for failing the mini-game (Possibly after 2 or more failures) would be good. With the addition of point deductions and new text being good additions. Though winning at the mini game should offer a reward as well. Lastly, in terms of future development a chance for Kazuma to redeem himself or win through the mini-game. It would also be a good idea to come up with some faults for John to add on why Kazuma should be able to win.
On a final note you've stated you are thinking about how hard it should be to avoid NTR and to that I suggest that it should be very hard. For the simple reasons that there is a planned easier route. I believe that to avoid a NTR take down should require the player play the RPG section efficiently, but also that the player does the character interactions and mini game near perfectly. Ideally the average player should lose deep into the main story or after multiple adventures. I believe this would build the most enjoyable NTR experience, as the player would lose not because they chose to but because they didn't do a good enough job playing the game.
