Genuine question, does Betty get slimmer somewhere down the road? So, any word on the path with the sexy Italian neighbor? I feel like it was one of the early storylines that was started and very little has happened except us going over there once but that didn't really further the story much. Or did I miss something?

Right but that particular cuck is French. Not Italian. And yes the name is shared between those cultures.
The Italian husband is a cheating, abusive asshole. Which if there is an Italian stereotype in the sexual sphere that is certainly one of them. Not physically abusive (to the storyline knowledge of our MC) but degrading and demeaning without consent. Which is a stereotype but also not an inaccurate one unfortunately.
Hopefully in the Italian storyline with Natalia the husband will get hoisted on his own pitard and get his comeuppance and sure be smaller than MC (cause plot and divine dickness) but otherwise his hook keeping her was his abilities in bed.
I could see some Italian storyline happening post-Paris. Something something Amy meeting Italian hubby and he being charming. Amy, Thomas, and Natalie stinging Hubby into the dirt thus evidence of cheating wrapping up that line with a bow. +1 to Hotwife Harem and +1 Amy, But whole storyline is avoidable so obviously if not doing Nat's line isn't major. Make good use of Smartphone time...
I could also see Suka being a part of that if she wasn't as skittish as Molly maybe a way to weave Aiko or Asian Mother into the greater weave of plot if Amy isn't the line.
Also love to see more of the Mother Daughter Jaya and Alyssa story but I know MoonBox is doing their best even if that line hasn't gotten a lot of chatter or love in a few updates. Maybe that has the not pregnant Tayna line of story in it as it ties back to Thomas directly... Or even pregnant Tayna for those on that line.
Shame I haven't gotten more Polly or Sister Maria yet but I'm sure that's also in the pipeline. I see Polly's being complicated with Betty's line as apology for shoplifting if Betty is store owner (and also on Betty's line)
We did recently get some Redheads and Orphan line love. Also recently with Mabel some Grandma love. Some Bratty love with Piggy Anabelle which I can see evolving to Cow...
And yes Moon, if you need hands writing or bouncing storyline or even an extra hand I'd be willing but need to brush up on my Python... I'd want to help with writer's credits first before helping with code... But that's like. My opinion and likely you don't need the help or have plenty of other trusted hands first.

Which choices? I know the Fiona one confused some people but otherwise they seem pretty clear.Moonbox, the choices are not all that clear in this VN.
So, after a day of Tommy Boy lying in bed, I figured I had missed some content. I got the walkthrough mod.
HOLY SHIT, i had missed so much! I thought i had opted-IN to everything, but some choices throw you down pathes not so obvious from the wording. If this is by-design, it is an... interesting... design choice.
I will add this to my list of criticisms for this game (albeit minor; I love this story)
My issues:
-Unclear choices.
-Tommy's neck hair line (I'll never let this go, but I totally should. Its not keeping him from any pussy, sooo...)
-The garage with half roof, grass floor, car parked sideways, and blends into the living room. (This bugs me more than it should
