I've done it before, and yes, there's is a fairly large difference. For the most part the game still plays the same. But I was actually surprised by just how many of the storylines change whether your low or high.
That said, I feel like the low-suspicion content is kind of lacklustre when it comes to the election.

I just went through some more playthroughs and despite my best attempts to replicate some of my choices, it's almost impossible. I can't have the Chief of Police come to me to get bimbofied and I keep getting Mayor Justine chosen as my candidate, even though on my first time I had Sandra Otterman with 100 Bimbofication and won the popularity. Dunno if there's a way to increase City Bimbofication to max, your own popularity to max and also decrease opposition to 0. Cheats don't really work in those three areas. Even one single choice changes the whole game and I don't even know which one.
While it's amazing how much effort pCaine has put into this, truly, it makes it difficult to get your progress the exact way you want. I'm thinking sharing some choices and stuff can help us maybe build a sort of walkthrough. This game is one sexy maze I tell you.
For example, one of the girls Anne that comes in the beginning, has options to suit her parents or her own wishes. Choosing her parents and then turning her down when she comes the second time allows for the MC to buy out some land and start some expansion during the Hospital Endorsement. To get the guy to talk to you, it's also good to have Lorri on your side and choose the creative option. She then fucks the whole staff. You can also choose the first option, don't know what that is. I think that's for Sandra's route.
If you happen upon the Justine route, there's a "Help the ex-husband get a new wife" or something like that which will secure your Hospital.
Also, choosing the inexperienced nurse at 75k allows you to bimbofy her and the whole smaller hospital you set up.
When it comes to the technician, if you wait and see what happens, instead of immediately taking her to the machine, she changes into a different character although no less slutty. Don't know how that changes things as she pretty much has the same role.
I know that when you completely brain fuck your first lawyer, you have a chance at getting one of two different ones. First one looks middle aged, short black hair, but I didn't really explore her as much. Second is a perky blond that you can partially or completely bimbofy. First option, she remains slim, just gets a bit sexier and has two extra scenes during election time where she goes on a sex binge. Second option turns her into a curvy big breasted chick and there she has access to more bank scenes, but end result is 25k and no more bimbos from the Bank. Still unsure if that is based on the different Mayoral candidates or on the levels of bimbo.
The P.I.'s you hire also have 2 different routes each for each candidate, but again, I'm not sure how to control those routes. Keep getting different results.

If you choose to wait and see, she will change into Diamond Jackson the next month. In the plot, she makes that decision herself. Otherwise, if you put her in the machine right away, she's the normal Jada.

No clue. There are only 2 options as far as I'm aware. Maybe it's for the next update? Or maybe not. I haven't found it if it's implemented.

I did low on Summer, told her no we need to run a business, had Debra look at the machine and checked on her progress before answering the e-mails, had Mrs. Rocks show off her body, and I told Debra to wait and see, then I got the third transformation.

From my convocation with the developer about the Diamond transformation: you need Summer to be fully bimbofied in Month 1. Talk to summer about Debra then check on Debra's progress with The Machine

I see that some people are interested in choosing the other mayoral candidate that is not Justine. So I am writing my current playthrough and the choices I have made in order to go with Sandra Otterman during the elections. Keep in mind that I am cheating in this game, so while I don't have a certain answer on how to achieve the results normally, I will leave hints on what I think might have what reaction. Ultimately however, it depends on you if you want to test it or not.
Playstyle:
* Almost always choose the bimbo and perverted options.
* When prompted, always choose to have sex with clients who want it. Not sure how that may affect the playthrough.
* If you press the wrong choice, you can use the Back option on your browser to go back in the same way you would as if you're browsing a site. Then you can choose whatever you like again. There's a limit on how far back you can go, but it's still useful.
* Pay very close attention to your employees and always make sure to do their mini-quests. That's the way to progress with the second route. You will understand in detail later.
* If I haven't mentioned some emails or quests, then I don't believe they have an impact on our choices or give any bonuses. Please correct me where you think I'm wrong.
Cheats:
* Press F12
* SugarCube.state.active.variables -> look for Cash, change value to whatever you want.
Prologue:
* Check Mail
* Science fair invitation
* Go to Science Fair
* High for Summer
* Shut the door for the Bank Manager (I don't think it matters)

* Ask Debra to study the Machine.
* Free up time for her.
* Check on Debra's progress.
Month 2:
* Call P.I.
* Test the Device on Cathy. Doesn't matter who you choose. The second lawyer has some extra scenes though in each of her transformations. Personally, I prefer to bimbofy Cathy and then when I choose to use the machine on the second lawyer, I pick the first transformation. It leaves her with her slim figure, but it later gives her some interracial gangbang scenes that I find pretty hot. Her second transformation turns her into a big breasted full bimbo and she has some scenes with the Banker, but they are not that satisfying as at the end you receive 25k cash.
* Check on Debra and choose to offer her help. That's Jada's transformation I think this is the first step to the Sandra route. You will understand later.

* P.I. catches the thief and you can bimbofy her.
* Do the interview and tour. -> Give her the tour. -> Just a short treatment. Low suspicion + I think her actress is sexier.
* Email 1 Meet the Professor. -> Get her back out there. Low suspicion + you get to bimbofy her student in a later month.
* Talk with the Police Officer. I don't think it matters which you choose, since it ends the same way.
* Email 2 Special Request (IMPORTANT) -> Suit her parents wishes. Second part of this encounter will determine the progression of a later quest. It will allow you to expand and bimbofy the Hospital during the Elections.
* Email 3 Suspicious Email -> Have Debz look into it. -> Have Lexi look into it.
* Lexi has a problem with an ex-colleague. -> Meet with him. -> Sex scene + no suspicion = no trouble later.
Month 4:
* Nothing much to do here. Most of the progress is linear and there aren't many choices.
* Debz request will net you permanent incrimination should you choose it. That can be fixed with a cheat I will show you later, so it's you choice if you want to see some sexy gangbang pics.
* Summer's idea can give you an extra Pink Lady if you choose "Let's not do anything crazy". One of the few times not fully bimbofying women rewards you for it. Otherwise you get a hideous monstrosity of epic proportions to shine on you screen.
* Looking through the Skinsational Sanctuary's Bell and choosing the second option will give you some sex gifs. Oherwise, just a buttplug gif. I don't think it matters much what you choose here.
Month 5:
* "Deal with her." option gives you a new Pink Lady and some lesbian scenes.
* For the new employess, I had chosen Jewel during my Sandra playthrough. Not sure how that affects the route.

* Cathy's gym problem. -> First option does nothing. Second option gets him to go away + sex pics.

* Crystal's email. -> Choosing Paige gets you 2 pornstars. Faith only gets you 1, herself.

* Lexi starts the investigation quest. Make sure you've completed everything else before then, just in case. -> Go back to Lexi again. Talking to her old partner. -> Choose second option and eliminate some evidence.
* Talk with Debz (IMPORTANT). -> Hacker scene. -> Something more creative. -> This allows you to bimbofy the Chief of Police and the station, secure the entire Police Endorsement during the Elections and is also what I believe to be the second requirement for the Sandra route.
Month 8:
* First, go to Abi and check out her request. -> Choosing the first option, gives you some sexy scenes. Second option gets you a Pink Lady, but a permanent incrimination. Can be fixed with a cheat. Will show it in a bit.
* Talk to Lexi starts the second part of the investigation quest. -> Each choice bimbofies two women. First choice is his girlfriend. Haven't explored the second option, but it bimbofies some photographer. I had the first option when I did the Sandra route so keep that in mind if you don't want to experiment.
* Check your emails. (IMPORTANT) -> Daddy's little BJ slut returns. -> Send her away. Secures the bimbofication of the Hostpital and its Endorsement. Otherwise, you'll have to pay the owner 250k and I'm not sure if it will help.

*There isn't much to do here. Just finish your To-Do list. -> Renovate the porn studio. -> Porn studio party.

* You should have finally unlocked the second candidate Sandra Otterman. Go look for her at the Campaign HQ.
* Meet and treat Jordyn's candidate. -> Choosing the second option starts the City Bimbofication route. Don't know what option 1 does.

I've got it! It's a virus simulator game! Like, you know Plague Inc. where you have to develop a virus that will kill humanity. Here is basically the same 
Which Mayoral candidate you back depends on how well you handled the investigation:
If the investigation is called off, you bimbofy the reporter and get Sandra as your candidate.
If you screw up the investigation, you bimbofy the judge and get Justine as your candidate.
Tons of shit play into the investigation, though, so if you want Sandra, aim for her from the beginning with a low suspicion run.
One other thing about the election:
If city bimbofication is higher than 50% before you end the first month of the election, your opponent will start acting like a bimbo too at the start of the next month. Otherwise she will be a normal candidate
If the investigation is called off, you bimbofy the reporter and get Sandra as your candidate.
If you screw up the investigation, you bimbofy the judge and get Justine as your candidate.

The game is funny and is huge, for the kind of the game it is.
Being HTML sometimes one can miss the point that, if the same amount of text was put piece by piece in small dialog boxes, as in a Ren'py game, it would require thousands of panel to read all of it.
The static images... well, they come from real porn, so the girls are hot. And they don't feel too much as if stacked randomly, as often happens in other games which use real porn.
Of course, the whole game has a big tongue-in-cheek attitude.
The gameplay is interesting, albeit simple, and not distracting.
