Had a quick look at the code, I've only done an Erynn run so I'm guessing, but if someone can put 2 and 2 together
This image triggers when at the training camp, with the flag Erynn_Week3_Wednesday_Lying == True. Looking for that variable, I'm guessing it occurs if you don't break things off with Alexis for Erynn, and then when making out with Erynn and she doesn't want to take it further, you go to Alexis instead, and when Erynn confronts you, you accuse her of lying. This ends both Erynn and Alexis' path.
Like i said, i haven't actually done any other path other than solo Erynn, so i can't say for certain.

This happens when you get together back with Alexis and make out with Eryn after Ethan slaps her. And instead of training with Layla you go on a date with Alexis so when Eryn comes over she confronts you, in the end Alexis breaks up with you no matter what you tell her.

Now I remember.
This was the consequence of doing everything you guys mentioned between Alexis and Erynn. Then when Erynn and Lexi confront you in your bedroom, there were two choices: You confess to cheating on both of them which makes them both slap you and end their relationship with you in your bedroom.
And the other choice as mike12013 wrote, was telling Lexi that Erynn is lying which makes Erynn slap you and walk out. That choice leads to this scene where Lexi breaks up with you during the phone call. I guess Erynn is there at the bar because she told Lexi that the mc was lying to her about what really happened.
I think that's correct.(?)
