As an internet armchair specialist who tried to re-create the image irl, I would love to add to the discussion above.
The toes can spread out like that, but only a small portion of the population can spread all of their toes in different directions - most will have their small digits accompanying each other while the largest digit is independent. Especially if you're someone who's predisposed to wearing close-toed footwear - which is the uniform footwear in most places from pre-school to high school, some colleges, universities and companies. I prefer walking barefoot and so far I can spread my Big Toe and the two smallest toes apart but the two middle toes can't spread far from each other and they keep facing the same direction. There are physical therapies tho that work with the toes because of course there are - so that far apart isn't too far-fetched.
The arms can move there, but his posture's too straight and stiff for it to actually be comfortable for him (unless he got disconnected joints, which happens). He needs to lean back further or tilt towards the right to accompany the reach-around, otherwise he'll just be using her pelvis as a support to keep that posture or he'd be straining himself to put a small pressure on it without crushing her.
Remember, he's also a man, so his center of gravity is in his torso - ain't no way he's spreading his legs without support on his ass, without anything to hold onto, and still be able to maintain an almost straight posture. All the while he's having intercourse.
