RPGM is sometimes very badly used in certain games, it has traumatized many.
You get the option after youve gone through all of the beginners cave. Done all those "quests" and gotten a half moon thingy.
Thx, the game is locked if you refuse to save that child because you do not release this cave. I restarted the game and saved the damn kid, I hate being nice.

The cat keeps telling me to clear quest 37 and then I'll get a new quest at the port. I finished 37, and there's still no fucking quest

I'm almost in endgame and there's not even a trace of NTR/rape. If you intentionally lose against enemies/bosses all you get is the standard RPGM GAME OVER screen.

The EPERM error is related to the game having insufficient permissions on your computer to run. The easiest fix would be to make sure you are running the game as an administrator. If you are not then try that first. if that fails try moving it to a different location on your computer or try a restart of the computer as sometimes permissions can get fucked and cause errors.
Then there is the more pressing threat that cause this error. This can also happen if something has changed your read write permissions on the computer. this could be a program you installed that restricts permissions, or malware. if you are seeing this and all the above fail, you might want to check your computer for malware as something might have infected your PC. IT IS UNLIKELY THOUGH. It is more likely you need to play in administrator or move it to a new location.
