Haven't played it yet but if it works anything like Halloween, just move towards christmas in game (going to the office once in a while might be needed, maybe even working) and the game will lead you.

While there are instructions, its still a case that even if Emma is open to vaginal penetration in cowgirl or missionary, she's still in pain doing Reverse Cowgirl. So its probably a bug in the game.
