It's the special "multi-vitamin (facial) cumshot". You have to collect 10 multi-vitamins and in "Play" mode interact with the multi-vitamin bottle on the bedhow the games count the facials? i did a lot of kamasutra scenes with any girls that end in a facial but the games doesn't count them
