I'm guessing changes in the dialogue watering down the talk about her exes during her intimate moments with the MC. I didn't bother with the fix since I didn't mind the previous set of dialogues. I rather look forward to the next update for the romantic progress of Caroline with the MC. She's falling hard and fast I like it.

No, just load the LAST update (0.4c), extract and play !!! Your old saves works (no need to move them anywhere) !!!! Normaly letters in version numbers 0.2 (0.2b,0.2c) are just bug fixes, not the whole new updates !!!!

My guess would be to fix the excess commas, since the writter said before (in this thread) there was some kind of problem that resulted in them apearing too much in the middle of dialogue.

Changing a character backstory because some people disliked is a textbook definition of pandering.
Also a quick look at this thread will show most people weren't bothered about Caroline having boyfriends and even between the complaints most were more about the wording (how she talks about them during sex) than the fact she had plenty of previous sexual experience.

The Patreon model is different than anything else. It's a continuous payment plan where people decide based on the updates whether the product is worth the regular payments or not.
Similar to some video games that are released in beta stages in order to gauge the reaction of the fans and make changes accordingly.
