You need to buy a rat trap in the shop and then go to the homeless guy (I don't remember his name right now - Jamal?). There will be a mini game in which you need to spot the rat.

After that you select anal lube in your inventory and after click again on anal hole.

To unlock the others two you have to use that first ... if there is no some bugs.
I have the same result ... but some depends on our choices during the game

Probably it is a bug , search in this thread for a solution.
Are you running a clean 0.7.1 ex version ?
If you want you can try my save (cowboy) that unlock other outfits
