Just load the save and play; it will ask you at the end of the 0.30 content if you want to see the spin-off, just like it does all through the story for prior spin-offs.

This is a Holiday Special version, try to read what the game informs you sometimes..
It's not part of the game's story as such, also all choices will inform if they lead to a lewd scene or not and hence won't need a Mod.
The I Patch at OP works for this version, can confirm that.
In the Holiday Specials, there are 3 now, the game assumes you.. and all girls/women, are fine with each other.. you get a choice to see the lewd sex scenes or not and that's it. It won't affect your main game story.
Holiday Specials are shown as Stars in the replay function.

Some games opened very slowly, or wouldn't open, on double click. I found if I use Right Click/Open, they always do. Doesn't make sense, but that's what I've encountered.
