It's very simple...The best NTR games are the NTR games that tugs your heart string such that you personally fall in love with the heroine. Her kindness, her strength, her beauty. All of those should resonate with the player and make you want to root for the heroine and the MC.
I don't consider cheating games NTR. A woman who cheats is just a moral-less F-Tier woman. It doesn't even matter if she's hot as fuck to me. But I do like a challenge when the villain is hot as fuck. It completely breaks immersion to me that my ideal woman would ever fall for a fat sack of smelly, piece of shit.

I don't really fixate on that much, well there are limtis mostly on the ugly because i can't fap if i have to burn my eyes after seeing untold horrors, for me is more of about the character of the Netori guy, i don't like it when they are nice dudes, for me ntr to feel really like ntr the guy needs to be bad, a villain or at least very self centered compared to the MC.
