I'm not here to criticize anyone's favorite games—sometimes even flawed ones have standout features that make them special for certain players.
I've played a wide range of highly regarded titles, from Japanese to Western devs, and in my view, people tend to enjoy:
- Gameplay-driven NTR events: These occur when you mess up or fail skill checks, leading to NTR events or gaining NTR points.
- Natural progression in a realistic environment: Avoid shortcuts like "lewd crests," "he has a big cock, so I must suck it now," or "my husband is sick, so I need to suck 20 cocks." No instant corruption over one night—at least let players witness the gradual corruption through repeatable events that start innocently (like talking) and build to lewder ones. These can be hidden or open, but they should feel organic.
- Male POV focus: I personally don't vibe with NTR games featuring a female lead, as they often just feel like standard corruption games. Or if a female-lead game has an NTR tag, it's rarely about genuine cheating on a loved one and instead devolves into "fucking the whole village" with dual protagonists.
- Discoverable sex scenes: You have to uncover your Love Interest (LI) cheating on your own, rather than having events auto-trigger once NTR points hit a threshold.

Lately, I've noticed that even with these popular NTR tropes in mind, many devs stick to overused motifs. Things like lewd crests or viruses that corrupt your LIs overnight without tying into gameplay, or super simplistic choices like "Leave her (NTR)" vs. "Don't leave her (Love)."
TL;DR for NTR fans: Do you find it acceptable to have so many simplistic games where your LIs fall after just a few minutes of play? Or do you mostly play them for a quick fap?

Personally, I think a lot of devs try to go too big and add too much in one game, it just turns into a chore to play. Having too many possible antagonists for one girl just makes it a pain to play, more so since replays dont have the impact the first run has. This also happens when they try to add multiple types of NTR in one game. Either I end up on a route I dont like or I have to specifically pick the route I like, which also ruins it in large part.
As for your question, the FMC should have a good buildup before she falls. But have a sub heroine that falls quickly just to add scenes while you build up teh FMC. It can also distract the player so they might not see when the FMC falls, and gives a good hit. The noises you hear of the sub heroin were in fact the FMC for example.
As for my personal preference, I prefer teh sub heroin to fall to the same antagonists that the FMC will fall too, it just helps build up that antagonist. But having small mini NTR events with different girls and antagonists can keep things hot while you do FMC slow build up, just dont over do it.

For me personally, the biggest thing you can do is make it so that if you have a points system of any kind, make it actually matter, so that if (for instance) I go too negative in a relationship, the love interest doesn't give me an option to win her over or a screen comes up that says "are you sure you want NTR hur dur?", just fucking cheat on me already goddamnit.
I'm sick of games where your character is a complete dumpsterfire of a human being and yet the women give you every change in the world and even if you're at -100 reputation/love/whatever, the game still checks with you as to whether you're okay with them cheating on you.

You will have to meet weekly quotas to not fail at the game and those weekly quotas will share side quests resources with some girls.
The world will be build in a way that it can live without you, so skipping time will maen u missed babysitting events that progress NTR without you.
