The incest patch feel really nature and make a little more sense in some dialogue but not that much.
Basically, depends of your preference there but it doesn't change many things.

For me it felt more natural without it, as it made more sense to me how the relationship with them evolved. And as someone said, there's none of that roommate/landlady nonsense. So I'd generally recommend not using the patch.
But if incest is your kink, I think you should definitely go for the patch. It doesn't make the story forced or weird, so if it's your thing you'll probably enjoy it more with the patch.
