To be fair, I normally read the negative review rather than all the positive ones. Usually the "harsher" reviews are better for decision-making than all the gushing 5-star reviews, mostly because those glance over issues without explaining them well.63 reviews... most of them positive with 4-5 stars and people read only the 1 star ones to judge the game before playing it

Did you just go to the captain's quarters? IIRC, visiting the room doesn't trigger it. You should be able to click / tap the actual question mark to do that event. (At least, that's how I believe it's always worked for me.)
Not too sure where you went wrong. You need to see Nyx video game events and her medical exam before this one becomes available. You can send me you save file and I can check what happened, I'd you'd like.
