This could be interesting. As it is, I feel like Eva doesn't quite punish certain choices with regards to sex, or at least give some consequences that can spiral out of control.Those are some interesting points you've brought up. I wonder if the chance she'll turn the tables and screw Ian over will be more likely if Ian pushes his dominant role a little too far, too quickly; by that I mean, he humiliates her publicly, jeopardizes her position, or ousts their relationship to someone like her husband, before she's fallen too far under his influence. That could be quite interesting, and a warning not to get too arrogant or cocky or suffer the fallout.
I don't know if that sort of thing is seen as counter-intuitive to the game's genre, after all some people might decide to quit the game if they feel punished for exploring sexual options.
The only specific choice that backfires in GGGB when it comes to sex, that I can think of, is if you involve your girlfriend Eva in the BBC party to which she becomes a BBC slut if you let it progress to the point that she participates in the orgy. Both with Arthur and the wine, and with the gang, choices that you take that have consequences for the route take place outside of the sex acts themselves.
