When you switch back to the wooden box, it is destroyed and a grenade launcher is revealed. You have to QUICKLY get it. Then either dodge the bullet stream and then get to the circle or quickly get to the circle. Press enter/space bar to shoot the motherfucker down ! The arrow moves pretty quickly. Ps. I copied it from the available Paradise Beach Guide v0.2 there it is well described so there was no point in explaining it to you in my own words

Even if you have only done it on the back, you should have heard a sound and a comment from the girls, unless you play without volume, but you would still see the comment. It's the same mechanism as when you give Sam an oiling. If you want to do it methodically, start with the upper back of one of the girls, for example the upper left corner of Shirley's back, move the hand to the right, when you finish that line and reach the other corner, move the hand down one square and now move to the left, and so on until you cover it all, (back, buttocks and legs) then do the same with Mrs. Shelby. It's not rocket science, just a matter of coverage.
