Fairly certain it's near the end of the harem route and you go to a massage palour.
I'm kinda all over the place between my save and might got something mixed up.
Could always use this if you want to view the gallery. Might help you figure out how to get to it during game play.
Parental Love 1.0 Taboo + Gallery unlocker + achievements spy
While it should work without extracting the .rpa, a clean situation is always better.
If you do not extract you can have some errors duplicate ... exit and reload
Read the game changelog carefully ...
1 New folder , extract the game
2 With unren option 1 extract archive.rpa, delete archive.rpa from the game folder
3 Apply the Mod
Gallery unlocker, switch from only unlocked to view all.
Playing in this mode will also unlock the scene in the game because the persistent file is based on "if viewed"
You are warned
achievements spy, in the achievements switch from only unlocked to view all
+Fixed a bug with Ada gallery

That sounds like a serious trauma. You'd think it would play into the story more. I didn't even remember he had another sister.

It does - on the Sister only route. She is also mentioned on other routes briefly as she had panic attacks and Sofi has one.

Just finished the game after putting it on hold for basically it's entire development - goodness, what a treat! At first it was alright, but at a certain point there's just this jump in quality that really makes it a game worth keeping around. (Quality might've been increasing at a more constant rate than I realized.)

Too many pages to look back through so it'll take a while but I'm certain someone did find it during the harem path. If I figure out the exact trigger I'll try to "document" how.

Are we talking about the trans masseuse? Finished the game around an hour and a half ago and I distinctly remember encountering her on the harem path. Followed the walkthrough exactly, so I'm not sure what the trigger could be.
