It's been a long time since I played this, but it wasn't anything groundbreaking. Just the usual Good, Bad, and Whore ending kinda stuff. I think it was depended on your stats, but I don't actually remember it.
Don't expect actual "choice", this is a pretty railroaded experience.
(Honestly not even sure if it deserves the tag. Writing you this has given me doubts.)

I'll bite

Dame is the noble title equivalent of "knight" for women.
However a "Dame" has never had combat associations, it is not a term associated with warriors as "knight" is, and in the context of the game it is actually more clear to use a term like "Knightess" (although I would probably prefer "Lady Knight" or something) than it would be to use a title like "Dame" even if it didn't have its slang baggage from the early 20th century.
