OK here's an idea.
You play as an npc. You work in a coffeeshop and every day you just serve coffee to people as the order but you get to overhear their stories because THEY are the MC. All sorts of weird shennanigans occur. You could reference other games like have the mc and his sister come in and you hear them talking about how they have to keep their relationship hidden.

Scooby Doo in the Mysterious Multiverse
Shower thought of the day; Scooby Doo is a coward. Shaggy is also a coward. Scooby is clearly Shaggy's dog, so he probably picked up his cowardly behavior from Shaggy. So what if the other members of the mystery gang had adopted him? Then I realized that the show's formula turns this into bestiality porn once Scooby is Fred's dog. Fred has no personality, his only role is to send Velma, Scooby, and Shaggy off to places they'll be attacked by the monster so he can get alone time with Daphne. There is no family friendly way to show a ginger spit roasted by a man and his dog. So instead turn it into a 3 episode porn parody. Velma, Fred, and Daphne each get their own episode where they're either being sexual with Scooby or including him in their sex acts with other mystery gang members.

my idea comes from this youtube but if you want the tldr then this counselor is doing extreme measures to force hikikomoris out of their bedrooms and by force I mean stalking, kidnapping and such. they will be sent to mental hospitals, diagnosed with schizo and thus get their medical bills go through the roof. the comments are correctly pointing out how this is tyranny or someshit but personally what I found really nightmarish is the interview portion. the interview shows that the hikki is "on the path of recovery" or some bs but what I saw was different. and this is where my idea comes in
it's a scripted interview of the hikki saying he's on recovery but the truth is if he doesn't say these things then the bills will just go even further. and that's where it hits me. what if this counselor is the government? the government extorting money to what they view as leeches of society? they need those tax slaves to pay off the covid expenses? thus I dub this "the war of the hikki versus the state"
our protagonist is this counselor who in his blissful ignorance thinks that he's doing a good job converting hikkis into normies. but what it really is, is its just the government giving him priviledges to stalk and kidnap said hikkis. his job ends when he forced hikkis into the mental hospital and get his money from there. he also blindly believes that the mental hospital is reputable and all that flare although he heard rumors that says otherwise.

Was looking at some of the new threads here and had a brainstorm!
Comedic parody of typical pokemon style gotta catch 'em all harem games.
You play as the average horsecock mc and run around trying to get all the women in town, but none of them will do anything with you because they're all too afraid your dong will put them in the hospital. Most anyone will offer is a handy because they're not trying to break their jaws or risk internal hemorrhaging just to get you off. So you have to try balancing your time between work, hunting the hotties, and trying to find a safe way to shrink you meat to normal human standards.
Needs to have a strong emphasis on the humour aspect of it though. No drama or rivals or ntr shit. Just a lot of really horny but waaaay too scared chiks giving you the cold shoulder until you're not a qualifier in the Guinness Book of World Records any more. Could probably get around the no real sex with them thing by having MC fantasizing and dreaming a bunch about the gals he meets though.
