As far as I know nobody other than Hunter knew that Drew became a Chick with a Dick so that's what everybody but Hunter thought Drew would look like. If I'm wrong someone can correct me.

its a joke, the reveal that drew is a beautiful man, and then the next chapter we are told that the guy in that picture is just a the taxi driver (or uber or something), and the real Drew is a woman.

The younger characters didn't know about Drew's transition, but Ophelia (and probably Aliza) knew. Ophelia makes some strange comments in chapter 6, that upon replaying, are obviously foreshadowing Drew:

Advice for anyone waiting for updates. Forget about the game completely. Don’t look at teasers don’t look at shit, and then come back in 9 months and the chapter will be mostly finished. That’s what I did. And when you come back still don’t look at teasers. There was a surprising amount of content from the last chapter spoiled with teasers so when I played it felt short af despite being one of the longest.
HA! I watched the teasers as they were leaked here and I already forgot what all the teasers had in them.

Gunpowder cocktail and A Family Venture are the only 2 I can say are like PV, but not quite exactly. No game is just like PV.
Anyone here want to elaborate on why in chapter 9 that Ophelia acts like a complete and total bitch to the MC? Because she goes from being motherly to full on bitch mode in .5 seconds in a single scene. Honestly at that moment in mind I was thinking "I wish there was an option to stand up and slap her hard across the face".
She was in her "Femdom" Phase if you continue the Game it should stop in Chapter 10 if I remember correctly.
