I don't think who the father is matters when it comes to Radiance of Life. Each different father will give a seperate item as well tho.

I'm about to start a fresh new game, i want a bit challenging but possible to clear. Is Maniac only possible to clear in NG+ or it's also possible for first run as well? Or i should go Severe for my fresh run instead?

Damn, from all the talk this game is challenging and very demanding towards true ending and append. I'm not a fan of replaying games as I get burned out after one play through but after reading all those praises I guess this stuff is much higher than generic rpgm gameplay.

Any tips on this game that would help me to not get junked by mobs and progression?

Try CASUAL difficulty first, Despite the name, it's actually hard.

You can receive the prenatal care only when

1. Your pregnancy has progressed enough that Dina asks about it, and you answer her to keep your baby
2. The father is not someone that automatically puts you into a separate route (Atsuo and the 5 gigolos)
