how to start a new blank game? When starting a lot has happened already.
What do you mean?
If you mean why do we start with 2 Goblins, 1 Orc, 1 baby, and 3 women? With a lot of rooms? That is just how the Demo/Test build works. If you mean anything else, that isn't apparent. We aren't far enough in to have a 'blank' start.

It's also worth noting that this is practically the minimum required to actually see the game's current content. In older versions you could obtain orcs yourself, but those features have been temporarily shelved in the latest version so they could focus on other parts for a bit. It's presumed that these older features will be returning eventually in some form or other.

Ironically, "shelved content" like that is a reason to keep pre-release versions. MBM had it's own share of cut content and it's nearly impossible to find old versions except for 0.1 and 0.7.
