I have it and haven't yet finished Chapter 2. I thought it was a reward from a sidequest, although I'm not certain. If it's not from a sidequest, it might be from returning to the Chapter 1 Mountain Forest area to visit the backroom, or using the bunny suit in the forest to reach previously inaccessible areas.
EDIT: Just checked my quest log and it's not from a sidequest. There's also a bunny suit-only area that can be accessed from the forest backroom, maybe that's it?
Logged into my laptop and my save disappeared. Can't find it, don't know what happened but I do not want to start all over again. I was in Chapter 3 or whatever chapter it is where you can fight all those bird monsters. Anyone got a save they can share? I really do not want to restart from the beginning.
The fifth costume from the notes should be "Weeping Cherry".
From what I recall, it can be unlocked by working in the brothel with the heartache condition.
You should get it in a short sequence after triggering a cut scene.

After you unlock the last ending you can go to the gallery at the bottom where the endings are and in the last spot it says bonus. Selecting that will lead you to the "Confession Development Room". At the top there is a box with the last sword in it.

Not gonna lie, I completely forgot about that character... But I can tell you that I 100%'d everything without ever using her "hint", so it's probably not important.
The hint in Japanese is 縦 (Tate), which can also mean Shield... maybe she is telling Sakura that she should include a shield in her style that uses 5 weapons? I think it's just banter, not a real hint.
As for Weeping Cherry, I believe you have to WIN the heartache battle, which is essentially a superboss. You can also acquire it via the Development Room after beating the extra chapter (The Development Room can be accessed in the Gallery at the bottom right).
