I think the main download links already include the 1.2 patch... it's the "other" patch that is optional.
And as far as that patch goes... Nope. Quite the opposite. The patch adds extra (more extreme) content... like scat and bestiality.
That said, if I'm remembering correctly, there is some minor scat in the base game. Though honestly, I can't remember. I think it's more hinted at rather than in your face... but I seem to recall that it is very much part of the story arc and so I am left wondering if it's going to cross your "I don't want scat" threshold.
Either way... don't patch the game and instead just play it. If the content is too much... just uninstall and play something else. It's not like there's a shortage of good fetish porn games out there right now. (hint, hint).
This game fucked me more mentally than I fucked my hand physically...
I should've read a few comments before downloading it. And now I read there are different endings (had the "Escape" one) and I have to download it again - since I instantly deleted it afterwards - , because I can't sleep tonight if I don't get an at least somewhat "okay" ending... fuck me.

Not sure if you ever played Euphoria, which IMO is less brutal, but also fucks with your head. If that's still too brutal, you might want to try Gibo.
Edit: Oh shit. I'm a total idiot. Here's another one: Discipline, the artist of which is the same, and arguable probably the most like Starless.
