Mine are always set to locale UK settings, others Japanese. If you didn't, restart the PC for the change to take effect. Or you could use NTLEA instead to emulate it.

Could you give me a link for NTLEA? I can't find it anywhere. I'm sorry for troubling, it is my first time that i'm having this kind of problem.

I honestly don't know what else to do, I've already downloaded both NTLEA and Locale.Emulator.2.5.0.1
And the language option does not appear, in the case of Locale.Emulator.2.5.0.1, in other applications even the language option appears, but not in the game executable, thank you for any help =/

Locale Emulator doesn't work with a 64Bit game. I think NTLEA should just work how it's setup (unless I changed it for the old Japanese DLC installer) as default, it seems to automatically matches time zone...

Does the patch from Surgy still work? Or do I not need it? I remember I couldn't play because the load times between sexual actions was way too long to be playable.

Probably not, things like that usually need to remade for different versions of the game.

Is there any way to fix the long loading times between scenes and "etch" actions like kissing, etc...? Or is that how the game originally is? I've tried ntleas but even that doesn't seem to work.

Think it's just the way it is. The base game (although I mostly played the one from Surgy) seemed ok to me, but the DLC is noticeably slow loading some scenes.

Try NTLEA, think some people have had it work when actually switching didn't. When actually switching remember to reboot after, also some people used Japanese locale and time zone and it seemed to work (but I don't).
Otherwise maybe it's the file address, try moving the game somewhere else.
I have the same black screen problem and the locale is not giving me a promt to even click it, i can only start the game normally or trough admin which does nothing
