Sooooo the infection ... im probably missing lots of stuff, is the infection (black mouth?) related in any way to the MC and/or the uh voice talking to him? Is the voice (face) the same voice as the eye aka the origin? Im kinda confused at this point.
Is the MC infected or maybe even the origin of the infection? The infection has to be at least related to ella, right? And they (sin) need her for ... whatever ritual(s) ... while ella needs the mc as her chosen one.

The face that the MC has been seeing lately is the Eye, yeah. He remarks the first time he sees it that it looks different than he's used to, and it talks like it's the Eye, so they're the same being that has just changed. The Eye has said that its pieces are coming together, so that's presumably why it's a whole face now and not just an eye. It's probably the Eye's voice that the MC has been hearing, too.
The Black Mouth's infection seems to be unrelated to the Eye. It's some sort of biotech antenna, basically. A way for the Black Mouth to extend the range of his powers. Anyone infected by it, the Black Mouth can speak to and influence. If they reach the skin-covering stage, he can control them directly, taking over their bodies. He can then use an infected host as a relay, sending his signal out from them to control anyone with a lesser infection. We don't know where the Black Mouth got his infection from, but given that Yuren found it inside a meteor, it's presumably much older than the current conflict. Something the Black Mouth sent out into the universe hundreds of thousands of years ago.
We don't know what Ella's rituals are supposed to do. It's probably responsible for the eclipse, Doll Ella takes credit for that, but that's just the first stage and we don't know what the others are. It might have something to do with the Eye, in one of the Dead Ends the Eye is strengthened by the eclipse. But it's still one of the big mysteries of the story.
