This update addresses slowness issues from 5.4.0 and 5.4.0a, and those upgrading should notice a meaningful improvement. Please leave feedback if this is (or isn't) the case.
Additionally, this update fixes a routing issue when going down Sophie submissive path, wherein certain scenes weren't occurring.
There are several other minor corrections in the update as well.
While not required, it is advised to start a new game when playing 5.4.1. Loading a game from 5.4.0 will work, however some issues may still persist.

So um is my game bugged or there is no sex scene or anything in the end of sophie romance route? I can eat lunch in her office but can't seems progress if even there is an event after that.
That's how it ended up for me as well. Are you playing 5.4.2? I was wondering if the romance path got fixed/updated but I'm guessing not.
I have to admit that of all the games I have played, how the Dev ends this one will be interesting. If you turn your sister into a dog, she can get knocked up by a stray dog in the alley behind your house. How does that one end?
A lot of feedback has already been given, but I'd like to share my thoughts about things that I haven't read by other users on this board already.

5. Dosage. As a fan of the slow burn, I think it would be nice if there were more "steps of escalation" for every type of MCS. At the moment, I think it's ~ 2 doses until you have to switch for MCS-+1. I realize, that this is probably wishful thinking, because you'd have to create a lot of new material, but I think it would make it a bit harder to "speed run" this game and give the players more "meat" on the bones to sink their teeth in.
6. Outfits. PLEASE, PLEASE, PLEASE give us the option to just click on a set of pre-made outfits. Other games have that option as well, so I don't think it's impossible to implement. I don't enjoy clicking a few dozen times just to switch between casual and professional or slutty appearance. But I LOVE that you included reactions to your outfit. I had to laugh, when "mom" and "sis" commented on the casual outfit during breakfast. It's these little things that contribute a lot to making this game so damn great.
In general, I think this game has the potential to expand. We don't necessarily need 13 chapters by the end and the story doesn't need to be fast-paced. I think the best way to improve this game is to look at what is already there and just make more of that (if the engine even allows that): an even wider range of random events with colleagues and family, more scripted events (taking mom, Penny or sis out on the weekend could be fleshed out more and turned into a whole ... thing), more sexual acts (maybe the option to use sex-toys on others as well as yourself), more outfits etc...
Like I stated in my previous post - I think this game might be the best fan-made, text-based HTML game featuring RL-porn I've seen so far. I don't normally write extensive reviews for things porn-related.[/spoiler]

Yes, just don't spend a lot of time at high arousal. Early in the game, before you've dosed yourself some sex slaves, you can just masturbate in your room in the morning and/or evening time slots.
It's actually impossible to become a bimbo once Mom is dosed enough. Past 4 doses on mom if you want to be a bimbo at all you'd better be done with it by then, and lock the gender if you want to keep that look for any reason.
Incidentally is it possible to become a male bimbo somehow, or is it just female or trans?
