The genre list is great although pretty vanilla, are you considering adding some fetishes of sorts or do you intend to keep the game as it is?
Really hope the MC isn't the guy in pic1. That skull tatt with a bird on it is just about the stupidest tatt I've seen on a male, hell, it's dumb on anyone. There's some really shitty stock tatt images for this engine, eh.

But to answer your questions:
They're in an open relationship regardless, he just didn't mention it there. You'll get a better understanding of what that means (what's considered cheating, what's not etc) in the next update. I was thinking of letting it be open to the player's choice, but that would mean more branches for me to cover, more renders, etc, and for now I'm trying to focus mostly on the main/side quests, and get a good portion of the game's story done if that makes sense.
About breaking up with Sarah, that is a question that because of plot-related stuff, I can't answer right now, so you'll have to wait a little bit

Again, the idea is to (almost) all the content in the game to be optional, 'cause I know way too well the feeling of a good game being ruined by a kink you don't like. So yeah definitely not doing that here!

And about sharing/swinging/(And I'll add netorase to the bunch as well) they are all planned, but none are obligatory (and none of that shit where if you say you don't want it, It happens offscreen etc. If you don't want it, it will never happen

But as a quick answer: You need to get her at least a little bit worried if you want to go with her, otherwise, she'll think it's not necessary. How worried about the situation she is depends on what information you got from the guy who shot you.
