how does the patch work? do I have to re-download episode 4 or is it included?

The patch is just that, a patch to bring episodes 1-4 up to episode 4.1. It's a script.rpa that replaces the old one in your episode 4 'game' folder (for Windows).

The writing's good, most of the women are hot, but.... Jenna is not. I like her character a lot, but Jesus Christ, she does not look like an 18 year old; more like a mid-30s porn star who dresses like a complete slut but hasn't even kissed anyone. I'm sure I'm not the first person to mention this, but it's weird that this character could not even exist and the game would barely be worse off for it, considering she's referenced in the game's title.

Jenna looks like an 18 year old that pays too much attention to her diet, thus her appearance is perfectly reasonable. I know teenagers that look a bit like she does
....I don't know if that's a good or bad thing.
