In orden to beat Karen you have to pick the right move based on wich attack she uses to initiate her combo:
Slap - grab block
Kick - insulator OR guard (the one with insulator is a guaranteed KO so go always for insulator) if she puts her foot in your face then guard
Punch - grab block (i think)
Sword leg - guard
after suscefully stopping her combo attack her with any move to reduce her attack strenght or heal with a potion/technique

I can't beat Lilly the water 2nd boss on the 5th floor in her 4th fight no matter what! She just one hits me! Any cheat or anything I can use to just win already?!?!?!

Lily's 4th fight can't be beat. She matches your health, and stats, and just one hits me for over 99999 damage everytime!!!

Lily's 4th fight is an EX level.
In all EX level battles, the opponent is never at stat disadvantage, no matter what your level/stat is...the game will balance it to make them more powerful than you.
I beat Lily EX by maxing affection(beat her in level 1 battle, then talk to her and choose compliment and repeat till you get max affection)as this will make Lily seduce you than use physical attack during the beginning of fight which you can use to spam attack and(hopefully) defeat Lily before she ohko you.

There were few accounts which used to upload TOT gameplay on pornhub. I used to watch them as a walkthrough tutorial and to watch the scenes which i didn't experience in my gameplay.

To add on the other dudes post, play through Yvonne's submissive path but don't fully submit to her. In the "rehabilitation" part of her submission plot there'll be prompts where you can submit and refuse her and if you submit to her more than once you'll be hardlocked to a Bad End.
Well shit, I'm already long past that part. Guess it's not that big a deal.

Keep in mind I'm only referring to the final part of her submission plot, once you successfully cleaned Yvonne's boots she'll offer a "rehabilitation" for you. During this part you should only obey her once and do not drink or eat from the doggy bowls she gives, refuse to be her toilet, and do not sleep on the "brand" on the floor.
