The problem is the code is not built for the PC to have both bits at the same time. Plus all of the text is built for a "one or the other" approach. Refactoring the game for it is more difficult that Aika is willing to invest time for.

The whole idea of this is that the odds are really stacked against you. The 'easiest' way to win the game is to actually let yourself be fucked. As literally everything in the game intends to bimbofy, sluttify, humiliate, and feminize you, did you really expect anything else? It's a porn game, not something you inherently set out to 'win' to the best of your ability.
How do I use the debug unlock thing? All I've seen for how to use it is to 'run it' in the same folder as my game. But I have no idea what that means. Do I have it up at the same time as my game? Do I play through it? How do I use this?

Simply run the debug unlocker once, close it, then open the main game. once you're in the actual game (when you can move around the dungeon) you can type the command 'debug' (without quotes) to enter debug mode. Every once in a while the debug unlocker needs to be updated, just like the game version, so keep in mind that some commands may not work if you don't have the latest unlocker. The debug commands list can be found on page 6 in this thread, but I'm not sure if that's still complete at this point.
I get told when I type debug after doing this that I'm not a debugger and told to go to the patreon page.
I've done as you said to a few times now and haven't gotten any other result. Am I using the wrong version of the game? I'm using version 10 1.0 hot fixes. And I thing this is the latest debug unlock, 181011 Debug Unlock.gblorb. Does it matter that I'm using Git.exe to play?

I'm honestly not sure what the problem is then. I just tried my 181011 Debug Unlock file for version 181028W and it worked like normal. The Debug command activated debug mode, and other commands worked too. I also use Git, so it's not that either.
When you open the Debug Unlock file, do you see the text saying "Hi there, thank you for being such an awesome fan. October 2018 debug...." etc.?
Yes I see that text when I load it and it asks me right under that if I want to restart, restore a saved game, quit, or undo the last command.
