Up until recently you needed to slap her to get her transformed - now it's kicking (and maybe kneeing). Could be that.

They will probably allow people to get it next october (at least i hope so), they might even add another exclusive command (they haven't mention this at all, but it could maybe happen).

not even sure what you're referencing here could you share any more details?
on the same note there's 2 things I know of that I cant seem to get to trigger either if anyone can help;
1. the barbara portal bra event where she asks you to get rid of her debt by talking with the pimp, all that works fine but there's supposed to be an outcome where he instead asks YOU to wear the portal bra in barbara's place. Ive tried to get that working in a ton of different ways but it never happened.
2. The "anal urinals" feature that was added a few updates ago, I think I remember it working in the original update but i cant seem to get it to happen at all anymore.

Referring to this as per changelog: "Should you faint as a result of the actions of a Tentacle Monster on a floor where you've already destroyed the Tentacle Breeder, exciting things may result."
