I think part of the answer is what you put in the third paragraph of your post. Depending on choices made, each one of those occasions you list, including Ben, could be her first time. If you're going to include dialogue/thoughts that draw attention to it though, there could easily be a flag in the coding that triggers the appropriate line to suggest it's her first time or not her first time. Other games do this pretty regularly and pretty successfully.
I 100% agree with this, the update was solid but took way to long to deliver. I get the feeling the Dev did start to undertake a remake then realized how much time and effort it would take and abandoned it, that's just my gut feeling however.
