Haven't experimented enough to give you an answer. It's likely gated behind story, giving birth enough times, a random side event, or a combination of all the above. Games with stat tracking like this can be a little crazy when it comes to how much you have to grind for abilities like that, so I'd recommend just waiting until the full release to find out. Like you implied, there's a chance it isn't even in the game. Quickest way to maybe find out would be to unpack the game and see if there's a public birth scene included, but even that isn't a guarantee that it's accessible within the game.

You know, I don't think he's so much of an asshole, the real asshole here is the person who purposely enters a game made by an author that always makes huge breasts and complaining about breasts being too big. The character in this game has about average breast size for this game author, it's the "general size" he uses for most "main" characters.
