The mod is self contained into a game file. Download the mod, unpack it, and then click on the game file.

I got the Birthing Lesson finally:
1. Get pregnant, go to fullterm (press esc, it tells you what stage you're in)
2. Go to Casino, talk to noble girl
3. Work front of casino
4. Work arena (the noble girl later mentions that you going to the arena convinced her to get pregnant)
5. I talked to the noble girl again, not sure if this is needed.
6. Use Bed
7. Go to birth in-between church and thug house, anon0 shows a picture of it 3 posts above mine.

She will be naked there when you get there, talk to her to start the scene.

It won't let me work the casino when I'm pregnant, am I missing something?

You will want to advance the Casino (and Cossack?) plot(s?) through it's paces. I make it a point to talk to the girl in the casino each time I go in just in case.

I've try to unzip and redownload it but still not working
