Anyone here for the gay tag should look up Hyungry's stuff. It's Sims 4 machinima but actually gay and very well done.
Looking at the walkthrough it says "Trans Woman" but doesn't allude to anything that is actually gay (man/man) so it doesn't look like it, but I didn't bother downloading the game. I just looked at the walkthrough because as many people here know, the gay tag doesn't usually mean what it should mean.
So, there is quite a bit of trans content. MC helps the brother of the titled Homeless woman become a crossdresser (or "sissy" as called in the game). There are quite a few scenes with her (she changes her name from Ollie to Lindsey in v0.7, I think), but the first scene with Ollie/Lindsey happens when she's still a cis male. There is also a random trans woman involved in an orgy. And there is one scene where a cis man (for now) blows the MC. All of the scenes are optional, but there is no way to have sex with Ollie/Lindsey without putting her down the trans path.

Yes. There's a choice early on in the game where you can enable straight content only. If you do, no Trans content will happen.
