If you want a realistic story with realistic characters a harem is not easy to write. Those situations tend to blow up sooner or later, I don't think even Maya and Josy would be willing. Maybe for a time, but...Someone must have have played Sisterly Lust recently to come up with that idea? (I just played a bit.)
DPC doesn't clearly like the idea of a harem for the MC so that's likely out. Plus I get the impression he made the characters not keen to share (Might change with Maya and Josy later.)

Well, character interantions are somewhat realistic, you don't get that (or at least not very often like in other games) feeling of "ok, this is a porn game".As if this story is realistic!
I get what you mean though. Most harem stories really have to stretch reality to achieve that ending. In Sisterly Lust the whole (possibly pregnant) family (plus lovers, as if a mother and three sisters isn't enough) can move to an island where not much of anything is forbidden.
