The issue is that the game was supposed to end at chapter 3, which means that the dev has no more plans. So chapter 4 is/should be all about trying to please the patrons.

hurm for chapter 4 hurm i kinda sad little bit.. hurm 4 scene with no creampie i understand the story a least made georgia creampie or anal creampie... hurm.. i hope next update will be better than this....

That was my idea. Several reasons for this.
One, is Elena is very smart - perhaps too smart for High School which I assume she either breezed thru class or did poorly because she got bored. (She is similar to a girl my brother used to date for several years - although she was pre-law and not pre-med).
Two, as someone who can succeed easily, she would thing "what's next" in my life, and, given her moving off of serial relationships with dudes she is gravitating toward stability with F and Dee. This is reflected in the horse riding scene where she thinks about her future so intently that she does not react properly when riding and is thrown by the horse. (Btw, that thought process in that scene is a steal from the musical Oliver, song "Reviewing the Situation"
Third, it is so easy to fall upon the paths of our parents as a career when one finally has to pick a path. And having seen her dynamics with her father, she certainly feels that is he can be a doctor, then she is smart and capable enough to be a doctor.
Fourth, I wanted to give her an empowering career.

I've sent the "bring back the name your save option" to MDots.
At this point I'm afraid an Elena spinoff or even just more content would just be as vanilla as this game.

Alas, I too feel a bit as if this game is past its prime. Like Bilbo the Hobbit would say, the game feels more and more "thin, sort of stretched, like butter scraped over too much bread." Of course, it remains a much better game than the vast majority, but still, there is a feeling of... tiredness that has been creeping in lately.
My humble opinion on how I feel about a few characters at this point:
Elena: Verily, this character makes me feel really bad. She doesn't have much going on in her life besides MC and D. Whenever we get in her mind, it's almost always MC or D who are occupying her thoughts. It's the most pathetic character of the game, more pathetic than the evil characters who cross paths with MC and D. A shame because I do like this character. She is funny, witty and straightforward. But Elena ( in my opinion ) comes off as a very sad person who clings on the little things she has in her life to find a meaning to it, but the thing is this: what she "has" is not truly hers because she constantly has to enter MC and D's inner circle to get to them. I have difficult time feeling like she's part of the "gang" like Georgina does now. I do hope she will get a good ending. I want her to be happy more than I want MC to have his way with her. I don't know if that is good or not for a porn game.
Georgina: Sometimes I like her, sometimes I don't. To be honest, I'm still expecting her to reveal herself as a serial killer or something. She is too conciliatory for my liking. It's hard to explain. There is something off with her, kinda like the femme fatales in erotic thrillers. They don't show how crazy they are at the beginning but at some moment, however, while they're straddling a man for example, they suddenly take a knife hidden underneath the pillow and stab the poor dude right in the heart. I have a hard time trusting her. Just who is she, really?
Margo: I just see her as a filler character made only for additional sex scenes.
Cassandra: Same.
Jennifer: To me she is Elena, but thrown in the washing machine and left to spin for a couple of days, hung to dry under a scorching red hot sun. Needless to say, not my favorite character.
Graham & Olivia: I felt like there was something great going on at first, then they were left on a shelf and weren't as interesting as they used to be once they were brought back. There seems to be a lack of closure here but obviously we will eventually get it...
Martin: I have mixed feelings. I liked Martin as a character in the early chapters. Now he has become someone else, as if he was bodysnatched by some horny alien creature, and his sole existence in the story now revolves around his love for D. Where did that come from? It feels unnatural and forced at this time in the story... But then again it remains a sex game and these things happen all the time.
Rachel: She was quickly introduced and subsequently maintained as this insufferable harpy hell-bent on making MC's life a mess but recently she almost made a 180 degrees spin ( "Macho Man" Randy Savage might've called it a 360 ) and we can sense a door has been opened... Redemption? Love route? Threesome with D? Who knows. It feels once again unnatural but I am still intrigued by this "New Rachel" since it's perhaps one of the only characters that might surprise us in the future.
Koko: Well, at least we have something new here too.
MC: Let's be honest, we get away with things way too easily with this guy of ours, overall making our choices less important. Then again, do we really want to see these characters crushed by guilt and remorse at this point? Probably not... I think we all want a good end, even if some endings may be bittersweet.
D: Up until this new chapter I would've said she had become a walking sex doll but now I can feel she is a bit more self-assured and is overall a spicier, different version in this new chapter. Are we going to get in a real argument with D soon? Will there be an ending where she dumps us? Who will get her pregnant for real? Will we manage to re-corrupt her? That "no creampie rule" is quite the remarkable aspect of this new chapter. I don't think any porn game has done that so far... Like it or not it's something special and unexpected.

Money grab. Cash cow. Milking. These claims have been going on for over 4 years now. Most of that time I've been saying MDots wants to make the story he is making - with some allowances for fans - but still it is his story. For years it was the slow burn was a money milking. Now it's lack of other routes is a money grab. It's like a Q-Anon conspiracy based on belief of what is happening, but the reality the movie ends when the director of the movie films the last scene.
The other theory that does not jive with the money grab is the claim that MDots must be losing money because of the choices made in the plot, routes, etc. So is it a money grab - to maximize profit - or a bad business decision. And wouldn't it be a money grab if the story were plotted to please the maximum of fans and not ruffle people's feathers.
Or is it more likely, like I've said for years, that this story and style is what MDots enjoys and that is the primary driver on why we are now at Chapter 4 with the story proceeding the way it is proceeding?

I missed the planning memo that the game was supposed to end at Chapter 3. Where was that stated? The VN was never going to end until the fat lady sings - and, in this case, once you have My Big Fat Greek Wedding, then you know the end is in sight.

Actually it WAS planned to end DMD after Chapter 3 - but chapter 3 went too big after fleshing out the planned plot, so chapter 3 has been cut early and the rest of ALREADY PLANNED scenes were put into a new chapter 4.
So both are right.
But what no one wants to see is, that even there is now a chapter 4, the storyline doesn't change from the original planned one at all. Maybe even the opposite: the scenes will be more fleshed out than it would have been if all scenes would have been compressed into one chapter - probably even by cutting out some scenes.
So in short: Chapter 4 only has been opened to reduce the download-size of chapter 3 mainly
