Anyone have a complete save file? I've used the save file posted here but then all my other saves can't be used.

It is a little misleading, because the original version for the PC-98 (1992) had really short endings in the first place.
The Windows PC version (1999) actually expanded on the endings, so that change was a clear improvement in terms of giving the character relationships a more satisfying resolution. They did rewrite a few other parts but, in a few words, we should remember that what made this game so popular in Japan was the romance and dating angle, not the porn.
This was never a game with a lot of sex scenes in the first place, even in the first version. In short, this is for people who want more of a galge and less of a nukige. You're there for the emotions and the dating simulation, basically. Closer to Tokimeki Memorial (1994), except with a few ecchi and adult scenes.
That's why they were able to easily make ports for the PC Engine (1995) and Sega Saturn (1996), which were also famous versions in Japan and elsewhere, even though those consoles didn't show any explicit sex.
All of this is relevant because the 1999 version was the starting point for this 2021 remake. If you check the HCG sets for the Windows version, you will see they were very faithful to what is included in the current release (as long as you have installed the patch, of course).
You can always go back and look at a couple of extra HCG images found only in the 1992 release for the PC-98, in case you feel those are needed...but in terms of the actual playing experience, this one is better in most other ways.

I figured it out. The doctor only comes back when you've picked a wrong answer. I've been stuck too because I kept picking the same combination over and over. It turns out, even though there would only be one answer that is shown to be "bad" by the game, there is also only one answer that helps you progress. You just have to pick that one true answer each time she asks you something.
BUT THE MOST IMPORTANT PART, you'll have to pick the hurtful answer at last, she'll be mad and show you things ( .)( .) to prove you wrong.

still not finding out how to get the ending for the doctor/nurse. Can anyone help? I just can reach the part where I help out in the flower shop and she passes by. Hearts are full. But when I choose her ending on the 30th there is not really an ending scene since she simply rejects me. And in the CG there are two pics missing from her. Any help?
I think should be as easy as downloading the file from the Johren website and replace the files in your install location.
Okay now i understand what happen.The original creators ELF went out of business if memory serves me correctly around 2015 .They have a huge catalog of very nice popular games and hentai movies some of their employees now have their own studio and brought the rights of the games start to remake them and translate them into english. The only caviar most will be adult themed but nothing +18 or hentai due to be released on steam but if you spend more money can get a dlc with all of the goodies...
I will never understand the need to censor/cut parts of a movie/game when producing a modern update/upres. Keep it the same or add more, very simple but way too often ignored.
Sad, the production values of this re-release are very high and the artwork looks better too. Shame they didn't do all of it.
