To be fair I don't think there's any lewdity level in this game between "celibate" and "everything that moves and many that don't."

The empty spaces are for any pregnant npcs you may have. When the scene triggers, all those npcs will give birth as well.

you need finish the 8 level first and active the sister heart, you can get from the 8 level or use save editor to get the item.
