After trying to edit my review, it has dissapeared and for some reason can´t upload it again, so I write it here:
Really good and creative concept of a game and that´s why I give it 4 stars. The mix of medical simulator and erotic visual novel works surprisingly well. It has some problems, though. Without a particular order:
1. The design of 3D models is adecuate, but not outstanding. For example, both the MC and his sister has really strange hair, some characters are suposed to be in their 20´s but actually look 10 or 15 years older, etc.
2. Not a lot of sex and eroticism, actually.
3. It is needed a really deep medical knowledge to achieve the goals of the game without cheating, specially the medical jargon. The first patients are easy, but the dificulty increases a lot after a while. There are more than a hundred of medical articles in the codex that the player should read in order of playing properly the game. I´m sure a lot of gamers find it boring in an erotic game.
At the end of the day, the dev needs to to decide if this is an erotic game or a medical simulator where you have to deal with 12 y.o. girls with cancer, you see photos of bloody sputum or urine bottles, and need to know how to interpret the blood and urine test of an hypertensive and smoker 70 y.o. man in order of diagnosing him diabetes mellitus. Because the part of the children with cancer, radiographs and urine test is not erotic. At all. Actually, it´s a HUGE turn off. And it´s about 80% of the game.
In summary, I think this game would be a huge success in PATREON if it would have way more eroticism, tease, fetishes and sex, the part of the medical simulator where a bit more casual and kinky oriented, and the visual part where a bit better.

In fact I already decided what this game is since release 0.0.2, this is not a porn game at all.
If you look at how I introduce the game everywhere around, General Practitioner is "A different kind of Visual Novel" or "Medical Simulation Visual Novel with Adult elements", which I think describes it properly.
It has been linked on porn videogame sites but I never send them my files nor my press kit, so maybe that's why it's misunderstood. Some guy on Indiedb wrote almost an year ago "what is this porn bullshit doing here?", which I replied "Actually, this is not porn at all. Try it." or something like that.
Yep, medical elements and difficulty needs to be balanced, this is just a 0.0.14 after all, so that's understandable and you're right about that.
My game is an "Adult game", not a porn. It contains sex scenes, some are even explicit as we see in some movies which are still not tagged as porn on IMDB.
As for the characters, when I drew the MC I was starting to understand DAZ3d and I used the very old Genesis 2 model (Genesis 8 wasn't out), also because Genesis 2 is the only model with a doctor's coat available as well. Unfortunately lots of customizations for Gen2 are very hard to find recently, so I'm a bit limited about what the doctor can look, wear or do. With the sister I'm a bit more free and she's a fan favourite for her looks, but I understand why she might not be liked by many people.
Kiko, on the other hands, is a Genesis 8 model, and the difference is outstanding.

I see the dev chose the kind of game he wanted to do time ago. Probably the words "the dev needs to decide..." were a poor election. English is not my native language and not always express myself properly.
What I wanted to say is the elements that would make this game a bigger success in PATREON and the ones that prevent that some people (as me) pledge for it. In my opinion, obviously.
The main virtue of this game is the novelty and originality in a genre - the erotic one - that usualy lacks of originality, but an erotic (not necesarely "porn", understood as "sex, sex and more sex with any excuse) game with elements of medic simulator would be more "comercial" than a medical simulator with erotic elements. Again, IMO.
What I wanted to say is the elements that would make this game a bigger success in PATREON and the ones that prevent that some people (as me) pledge for it. In my opinion, obviously.

For easy and fast money I would agree with you. However, after this game will be released, which are the chances of making a name for my software house with products which are just aimed for adults? Look at how many people try their luck on Patreon with adult games. Of course many have 10 times my patrons and earnings, that's true, but many of them (including the guy from Big Brother) sooner or later end up in milking their patrons with updates which contain a single scene or two, no matter how well rendered, they put A LOT of farming just to unlock a blowjob scene or they leave their game incomplete to move on "something else" or even worse they stop developing at all (they already made money, so what's the point in completing a game)?
I'm talking about this not a a dev but as a player and "customer" and it's annoyingly common in Patreon's projects. I'm trying to make a game where the erotic element is present (because it's a very important part of our lives as well) but which is not erotic-centered. I would do way easier money with filling it up with incest scenes and other elements, but where would I be different from the others? Why should you give me money and not to another dev who makes the very same things I do with only different Daz models?
Because I want gameplay and story to be different, because I believe in this project su much that I left my job to focus on this with a wife and a kid and because I want Bruni Multimedia to become a recognized software house in the Visual Novel industry, no matter how much time it will take.
Making an adult porn game (idea which crossed my mind when I was starting the project) would've given me a lot of money, but probably would've made my next work "more of the same", which is not what I'm trying to achieve. I know I aim high, but I'd like my followers to think: "Wow, BM announced another game, I can't wait to see what they're goingo to do now!" and not "My God, this is still the same crap, let's subscribe to another dev 'cause he puts a lot of cracked buttholes which I like best."

As a medical student i thought that the medicine would be bullshit but I'm actually quite surprised everything is on point is the op a doctor or a medical student because he's very good

It can go to Steam now just like House Party, Dev just need to censor the sex scene and provide a patch for it and its good to go.
The game got's enough content now to go on Steam as a beta game and charge 10.00 dollars and they would go wild over it.

Truth is, roughly 80% of erotic games are mediocre, or just bad. It seems that, as you say, devs are amateurs looking for some quick money. But the fact that, even by making mediocre videogames they can get that fast money, proves that there is hungry in the market for erotic videogames. The level is so low that, when there is a product that excels in quality terms and profesional approach, it success.
There are a few erotic videogames made from a more profesional perspective that are making big money: "Good Girl Gone Bad", by Evakiss, "Summertime Saga", by Darkcookie, "Dating my Daughter" and "Melodie" by MrDots, Lessons of Passion, Vdategames... Those games and devs are not perfect, at all, but I think that they are making a name for themselves as developers of erotic videogames, a market niche that, until a few years ago, was exclusively ocupied by japanese hentai games.
Darkcookie is making +30.000 dollars every single month for several years now. He can even hire people and create a small company. For a product that is a semi-profesional erotic videogame! Most of the patreons are paying for this game the equivalent of the cost of three, four or more AAA videogames like "The Witcher III" or "God of War"! There is a big market for quality erotic videogames, that´s for sure.
My case about this game: I find really interesting to interact with the patients as characters: to try to gain their confidence, to find things about their lives (especially the spicy ones, as if they have the fetish of exhibitionism, as Kiko ), to flirt with them, etc. I would give more importance to this in the final game.
The part of trying to diagnose an illness is not as appealing for me. I don´t have the knowledge to do that and I´m not interested in reading a codex of more than a hundred of technical articles just to know the medical jargon. I´m not so interested in the medical science, I´m affraid. I would have preferred a more casual approach, without losing the scientific rigor.
In brief, I prefer the part of visual novel and interacting with the characters over the part of pure medical simulator, that I see as kind of a minigame too difficult to gain without cheating. The part of managing a clinic seems interesting, though, also as kind of a minigame, not as the main part of a game.
Just my opinion, of course. I hope you find it useful as feedback.

Nope, it's too early for that. The game is in alpha stage of development and very incomplete. I will have to carefully choose when to publish it because right now, even as an early access game, it's too buggy for a "proper" release.
That's why I chose Patreon, because the game is an alpha, people are not "buying" it but supporting my work. On Steam they would buy it instead and expect the game to be more complete, and that's completely understandable as well.
I also chose Patreon because of the monthly support over Kickstarter, because since this is a job I can live with what people are giving to me and I don't risk (actually I do, but in a smaller way) losing all support overnight forcing me to stop development to find another job.

The management part will become a very important feature in the game, even though I will make that non-mandatory for the story to continue since some people could not like that either.
I posted a poll some time ago on Patreon to assess the interest in both the story, the medical part and the erotic exams, and the percentages were almost identical. That meant that I had to focus on all three of them because players liked them all of them. I shall run a new poll to see if something's changed, after all being an alpha game means it needs help in balancing things as well, not only in sorting out bugs
As for the patients it has in the "to-do" list for a while to "spicy-up" things in story mode, for now it's in internal testing only but soon you'll have the opportunity to "interact" with them in a spicier way

You will get 3 side quest but none of them can go on yet and we will need to wait on them.
I think there all tied to the sister/cousin story arc but don't take my word for it.
We may even get more side quest but again we will have to wait and see.
